NHS Digital Weight Management Programme

This 12 week programme is designed to offer a personalised level of intervention to support people to manage their weight and improve their longer term health outcomes.

Services are for those with a BMI over 30* and diagnosed diabetes or hypertension (or both).

You will only be able to access this programme if you have a smartphone or computer with internet access.

*The BMI threshold is 27.5 and over for those with Black, Asian and ethnic minority backgrounds.


NHS Diabetes Prevention Programme (DPP)

DPP is a national scheme set up by NHS England.

It is a nine month behaviour change programme aimed at supporting people who have been identified as at risk of developing type 2 diabetes (those with non-diabetic hyperglycaemia).

The nutrition and exercise support programme aims to delay or prevent the onset of type 2 diabetes and those signed up can be supported face-to-face, digitally via an app or online, or remotely (via video calls).

Eligibility criteria:

Adults (18+ years) with no diabetes diagnosis, not pregnant at time of referral and at least one of the following:

  • (In the past 12 months) HbA1c between 42-47 mmol/mol or FPG between 5.5-6.9 mmol/l

  • 20+ score on Diabetes UK KYR score https://riskscore.diabetes.org.uk/start

  • Women with previous diagnosis of gestational diabetes and (in the past 12 months) HbA1c <42 mmol/mol or FPG <5.5 mmol/l


Specialist weight management programme (tier 3 and 4 support)

Further longer-term support is available for people who meet certain criteria, including a BMI of 35 and above. For more information, please speak to your GP or visit the Northumbria Healthcare NHS Foundation Trust website.


NHS Weight Loss Plan app

The free NHS Weight Loss Plan app can help you with healthier eating habits, to be more active, and start losing weight.

The plan is broken down into 12 weeks so you can:

  • set weight loss goals

  • use the BMI calculator to customise your plan

  • plan your meals

  • make healthier food choices

  • get more active and burn more calories

  • record your activity and progress
